1. 請(qǐng)假條
請(qǐng)假條可由別人代寫(xiě)，也可由請(qǐng)假人自己寫(xiě)。無(wú)論采取何種形式，其內(nèi)容必須包括請(qǐng)假事由和期限，必要時(shí)還需附上醫(yī)生的診斷或其他的可以證明請(qǐng)假事由的書(shū)面材料。
